Worldwide administration of messenger RNA (mRNA)-based vaccines for coronavirus disease 2019 (COVID-19) has resulted in a growing body of literature documenting increasing cases of de novo and relapsing glomerulonephritis following mRNA COVID-19 vaccination. Previous publications predominantly described glomerulonephritis after the initial or second mRNA vaccine administration, whereas reports of this complication after a third mRNA vaccination are presently limited.
A patient who received the third dose of an mRNA COVID-19 vaccine developed rapidly progressive glomerulonephritis; we present this case here. Due to anorexia, pruritus, and lower extremity edema, a 77-year-old Japanese man, with pre-existing hypertension and atrial fibrillation, sought evaluation at our hospital. Two COVID-19 mRNA vaccines (BNT162b2) were given to him a year in advance of his referral. His third mRNA-1273 COVID-19 vaccine shot was administered three months before he planned to visit. At the time of admission, the patient displayed severe renal impairment, characterized by an elevated serum creatinine level of 1629 mg/dL, a considerable increase from 167 mg/dL a month previously. This prompted a prompt decision to start hemodialysis. The urinalysis findings included nephrotic-range proteinuria and hematuria. Upon renal biopsy analysis, mild mesangial proliferation and expansion, a lobular appearance, and double contours of the glomerular basement membrane were observed. Severe atrophy was evident in the renal tubules. Immunofluorescence microscopy studies indicated a strong accumulation of IgA, IgM, and C3c within the mesangial regions. Electron microscopy revealed the presence of mesangial and subendothelial electron-dense deposits, which led to a diagnosis of IgA nephropathy, displaying characteristics akin to membranoproliferative glomerulonephritis. Examining the connection between pre-treatment serum parameters and best-corrected visual acuity (BCVA) outcomes in patients with macular edema secondary to retinal vein occlusions, encompassing various subtypes, after intravitreal ranibizumab or conbercept treatment.
In a prospective study at Heibei Eye Hospital, 201 patients (201 eyes) diagnosed with macular edema secondary to retinal vein occlusion, and all receiving intravitreal anti-vascular endothelial growth factor treatment, were enrolled between January 2020 and January 2021. Prior to the initial treatment, serum metrics were assessed, and correlations between best-corrected visual acuity (BCVA) and four parameters—platelets, neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and monocyte-to-lymphocyte ratio (MLR)—were scrutinized to pinpoint factors predicting successful intravitreal injection outcomes.
A statistically significant disparity in mean platelet counts was observed between the effective and ineffective treatment groups for RVO-ME (273024149109/L, 214544408109/L, P<0.001), BRVO-ME (269434952109/L, 214724042109/L, P<0.001), and CRVO-ME (262323241109/L, 2092742091109/L, P<0.001). The platelets cutoff value was 266,500; concomitantly, the area beneath the curve was 0.857, and the sensitivity and specificity were 598% and 936%, respectively. Women articulated that negative experiences and convictions regarding vaginal delivery, the pain of labor, and doubts about delivery outcomes played a substantial role in their choice to opt for a cesarean section. Conversely, cesarean section procedures offer a superior level of safety for infants and enable women to successfully balance various commitments. Medical professionals find that using computer systems for patient care is a less demanding and more secure method for both patients and practitioners. Considering the perceptions of both women and healthcare providers, interventions for reducing unnecessary cesarean sections, including the QUALI-DEC approach, should be planned and put into action.
